Note:  Check the TABC web site to see if the above forms or the fees have changed.  The TABC clerk said that the forms could change the first of October, start of TABC fiscal year.

Special Three Day Wine and Beer Permit
• Fill out
FORM L-04.5 .  Use the completed 2008 permit as reference.  The second page must be signed and notarized.
• The 3rd page is the
approval form for temporary licenses and permits which must be filled out as follows:(see 2008 approval form for details)
  o Superintendent of Parks, Gilbert Smith approves the upper half of the form.  He is at the  Ramsey Community Center, 16003 Lorenzo, Channel View, 77530.  281-457-0694
  o Phil Sandlin, Chief Deputy at Clear Lake Courthouse signs the bottom half of the form.  He is a Rotarian.
• 4th page is

• 5th page is copy of the first page and signature page of IRS Form 990 for 2007 showing Federal ID# 74-6070354 • Next 5 pages is a copy of The Articles of Incorporation for the Club
• Need a Cashiers Check for $201.00 masde out to Comptroller of Publicc Accounts; no personal check, cash or credit cards allowed.

NOTE:
(1) 3 copies are required, the original and 2 copies are for the TABC and one the Club’s copy
(2) application should be submitted 10 days in advance of event

(2) application should be submitted 10 days in advance of event

It will take you about 40 minutes to get the Permits run on computer.  You will also receive two signs to post on each side of the pavilion about guns.  You will also receive 3x6 notices which must be posted.  The permit should be copied and one copy posted where beer is sold and one each where the wine silent auction is.
