• Fill out FORM L-04.5 . Use
the completed 2008 permit as
reference. The second page must be signed and
notarized.
• The 3rd page is the
approval form for temporary licenses and
permits which must be filled out
as follows:(see
2008 approval form
for details)

of Parks, Gilbert Smith approves the upper half of
the form. He is at the Ramsey Community Center, 16003 Lorenzo, Channel View, 77530.

Rotarian.

It will take you about 40 minutes to get the
Permits run on computer. You will also receive two signs
to post on each side of the pavilion about guns. You
will also receive 3x6 notices which must be posted. The
permit should be copied and one copy posted where beer is sold
and one each where the wine silent auction is.
